The following State Department medical forms for adoption are in PDF format. These forms are generally not easily available on line, but many people need them for their adoptions. Please feel free to download any of them, to be used, when you go to the child's country, at an approved doctor's office and turned into the U.S. consular office to complete the adoption. The chest xray form is only used if any exposure or previous positive TB test is known.
The below immunization form is only used on adoptees over age 15. For younger children, use the Affidavit By Adoptive Parent, to waive the immunization requirement. We recommend all adoptive families use this form, completed in advance, so your child is not forced to get a battery of immunizations, just prior to your departure for home. You can get these once your are back with your own doctor, and not in a foreign country with their needles and their medication.
